We want you to have an efficient and easy registration experience. We encourage you to complete the following pre-registration form, so you can save time and be better prepared for your visit to our facility. This
service is available for expectant mothers who are planning to deliver a baby at Mercy San Juan Medical Center. At your Obstetric (OB) physician office you may have received a mail-in pre-registration form. Please complete either the mail-in form or this form online.
Please note that to effectively process your pre-registration, we must receive your completed form at least 72 hours prior to your scheduled or expectant delivery date. If you have less than 72 hours before the time of your scheduled or expectant delivery, you may pre-register by telephone. Please call our Patient Registration Office at (916) 864-5893. The hours of operation are Monday through Friday, 8 a.m. to 6 p.m.
What You Will Need to Pre-Register Online
Before you begin the online pre-registration process, you will need:
- Your insurance identification card
- Expected delivery date
- Obstetric (OB) physician
- Personal contact information

By completing this form online, you are submitting your personal information to assist in pre-registration prior to your arrival at our facility. All information you provide is secure to protect your privacy. After you have submitted your online pre-registration form, you will receive a postcard in the mail as confirmation that your pre-registration is complete.
Please note that there are forms that will require your signature when you arrive on the day of your delivery. Please bring your photo and insurance card.
